We are looking for people aged 50+ who have suffered from a stroke, COPD, pressure ulcers or people who are generally frail/elderly, to take part in an in home interview for 1 hour.

Please confirm which of the following you have suffered from; Stroke, Pressure Ulcers, COPD? Or would you describe yourself as frail?

Do you take an oral nutritional supplement or Medical sip feed?

If so, please tell us how often you are taking this supplement, how long you have been taking it for and the reason why you are taking it.
