MARKET RESEARCH – Project Alzheimer’s Disease October
We are looking to speak to people who have been diagnosed with the early stages of Alzheimer’s Disease OR a family member related to someone who has been diagnosed with early stages of Alzheimer’s Disease to take part in an interview for up to 2 hours in London.

Please answer the questions below if you are a family carer of someone related to you that has been diagnosed with Alzheimer’s Disease:
Who in your family has been diagnosed with Alzheimer’s Disease?
What stage of Alzheimer’s Disease has your relation been diagnosed with?
Do you live with the person diagnosed with Alzheimer’s Disease? Yes/No?
Do you help the person diagnosed with Alzheimer’s Disease with their day to day chores, etc? Yes/No?
How often do you see the person with Alzheimer’s Disease?
Do you see this person more OR less than you used to prior to their diagnosis?
What medication does your relative take for their Alzheimer’s Disease?
Would your relation be able to attend the interview with you? Yes/No?
