How long ago were you diagnosed with Atopic Dermatitis or Atopic Eczema?
Thinking back to a time when your Atopic Dermatitis was most severe, what percentage of your body surface area was covered?
How many visits to a consultant dermatologist have you had in the last 12 months?
Which of the following types of medication do you use currently use for your Atopic Dermatitis?
Topical Corticosteroids such as creams
Topical calcineurin inhibitors such as tacrolimus ointment (protopic) and pimecrolimus cream (Elidel)
UV Therapy treatment
Oral corticosteroids such as prednisone
Immunosuppressants such as tacrolimus and pimecrolimus

Have you had a discussion with your dermatologist about immunosuppressant treatments such as cyclosporine, methotrexate, or azathioprine for your Atopic Dermatitis? Yes/No?
