We are looking for diabetic patients aged 36+ and their carers (if applicable) to take part in a research about Diabetic Macular Oedema (blurry vision). This research will include 2 elements – a 4 day video diary task and a 90 minute interview in Central London.

Venue	- London, W1
Date	- Interview date will be arranged to suit. The video diary task must be completed during the 4 days prior to the interview.
Incentive	- £130.00 including the diary and interview with no carer. £150.00 including the diary and interview with carer.

Have you been diagnosed with Diabetes type 1 or 2?

Please tell us if you have been diagnosed with any of the following eye problems due to your diabetes; Blurred vision, loss of areas of sight in the eyes, loss of vision due to a detached retina etc, cataracts?

Please tell us the treatments you receive for your eye problems i.e. injections, laser, etc

If you have injections into the eye, please tell us when you are next due to receive an injection and when you last had an injection.

Please tell us if you rely on support and help from someone else when going for eye treatments and appointments. If so, would that person be willing to take part in this study?
