We are looking for people aged 18+ who have been diagnosed with Ulcerative Colitis and have had surgery to take part in an online survey for 30 minutes.

Other than Ulcerative Colitis, have you been diagnosed with any of the following? Cancer (which you are still being treated for), Stroke, Multiple Sclerosis, COPD, Chronic Kidney Disease, Diabetes? Please specify.
