We are looking for people aged 20 to 70 who have been diagnosed with Diabetes to take part in a discussion for 45 minutes in Central London.

Have you been diagnosed with Diabetes Type 1 or Type 2?

Please tell us the type of medication you are taking for your diabetes i.e. oral medication, insulin injections, managed by diet only, etc

If you are currently taking insulin please tell us the name of the insulin and state whether it is a short acting or long acting insulin.

How often, if at all, do you change the needle in your injection pen device i.e. never, seldom, always
