We are looking for diabetic patients aged 35-60 to take part in a focus group discussion for 90 minutes.

What medication are you currently taking for your diabetes?

Please tell us if you have ever had any changes to your medication i.e. have been prescribed addition medications or changed medications from one to another. Please give details of these changes and tell us how many changes in total there have been.

Please tell us if you are underweight, normal weight or over weight. Do you have concerns about your weight? Please give details.
