We are looking for people that have been diagnosed with Hepatitis C to take part in a 3 hour in home interview.

Venue	- In Home
Date	- Monday 16th September – Friday 20th September 2013
Time	- Times to be arranged to suit
Incentive	- £150.00 including pre task

Are you currently receiving treatment for your condition or have you had treatment in the past that has now stopped or have you never received treatment?

If you are currently having treatment, please tell us the name of the treatment that you are taking.

Please tell us how you think you became infected with Hepatitis C i.e. body piercing, tattoo, blood transfusion, etc
